Vibra Healthcare offers an evidence-based case management program to help our members improve their health and access the services they need. Case managers typically are nurses or social workers. These professionals create comprehensive care plans that help patients meet specific health goals
What is a Case Manager?
Vibra Case Manager oversees the movement of the patient through the clinical treatment plan to achieve the best possible outcomes for the patient through the clinical treatment program.
Case Manager evaluates, plans, monitors, and coordinates options and services to meet an individual’s health care needs. Our case management team process strives to assure effective outcomes for the patient, consistent communication with the all payors (managed care companies), as well as appropriate reimbursement for our facilities.
The case manager ability to deliver the right message to our payors, as well as our hospital and medical community, is critical to our patients and families that we serve. The goal is for proactive early intervention with the all the individuals we serve.
Case Managers Focus:
Our Case managers provide an exceptional level of customer service to our referral sources, our patients, and their families. The case manager are knowledgeable about our centers, services, and products to assist our customers. The case manager are motivated to meet the needs of the patient, families and all our customer. We excel at being in front of the right people at the right time delivering the right message. This can be accomplished telephonically, face to face, or electronically. Vibra Case manager are able to articulate to the payors a clear and concise picture of the patient and the goals that will be met will in our facilities.
