Skilled Nursing Facility in Central Massachusetts
In a skilled nursing facility, patients with special needs are looked after by specially trained registered nurses who can assist with more complex care services. These are often patients who have sustained a serious injury or are recovering from a serious illness and require day-to-day help while they work to recover and regain their independence.
The Meadows offers short term rehabilitation programs like physical therapy and diabetic care, in addition to 24 hour long term nursing care. We provide transitional care, or, an extension of hospital care for when they patient no longer requires acute services but still requires assistance with day-to-day living. With our help, patients receive carefully monitored medical assistance until they are ready to return home.

Most people are eager to return home after staying at the hospital, and want to get back as soon as possible. While assisting our patients through the transitional care phase, we learn more about their home life and health goals so that we can create a personalized treatment plan that will focus on improving their mobility and ability to live independently. To do this, we need to take the time to sit and talk with patients and find out what aspects of their condition are causing the most discomfort. Once this has been determined, we can start the healing process by helping the patient feel more relaxed in both mind and body.
