Claude J.

Claude, a 65-year-old male, was admitted to Vibra Hospital of Sacramento with the following diagnoses: encephalopathy, agitation, respiratory distress, atrial fibrillation, and pulmonary edema. He was admitted to the medical floor on a ventilator and was receiving nutrition through a PEG tube feeding.

Claude had an extended hospital stay, complicated by fluctuating respiratory and mental status, on and off restraints, sedation, and inconsistent medical conditions. Initially, Claude was able to participate in Physical, Occupational, and Speech Therapies. He then had a change of condition and was moved to the ICU, impairing his ability to progress for many weeks.

After stabilizing, Claude was again seen by Physical, Occupational, and Speech Therapy and started on an aggressive rehabilitation program. He was weaned from the vent first to t-mist and was eventually decannulated. Through working with Speech Therapy, Claude was started on a diet and progressed to safely tolerating a general diet. Claude made strides with Physical and Occupational Therapy as well, progressing to getting in-and-out of bed, transferring, and ambulating with a walker with supervision. Claude was also able to work on dressing, bathing, and activities of daily living with improved independence.

On January 4, 2018, Claude was able to discharge home safely with his wife. We are so happy to have witnessed his progress and wish him the best at home. We hope that he continues to advance and succeed alongside his supportive and loving family!
