The average hospital is not designed to handle the demands of patients requiring long term
acute care. That's why Vibra Healthcare's Long Term Acute Care Hospitals (LTACHs) are
special. Our many services are focused on improving the lives and medical conditions of
patients who either need more acute care for their recovery or who are not yet ready to be
transferred to a lower level of care, such as a skilled nursing facility or other sub-acute facility.
Because our patients' conditions tend to be more severe, we accept referrals at any time of
day or night, 365 days a year, and we begin immediately to focus on providing the highest
level of medical care. Upon transfer to a Vibra Healthcare LTACH, each patient is assigned
an intra-disciplinary team that develops a personalized care plan under the direction of a
physician. Team members get to know each patient – and each patient's family. In fact, we
regard family members as vital components in the treatment and healing process, so family
education and counseling are high on our list of priorities.
We believe it is so important for loved ones to be informed, that we contact the patient's
family within twenty-four hours of their admission to identify the method and frequency they
wish to receive progress reports, treatment validations and other important communications.
